11 Northwell Health hospitals have earned the highest patient safety grade from the Leapfrog Group's Spring 2025 report, a national nonprofit that rates hospitals across the country on safety, quality and transparency.
Leapfrog releases a Hospital Safety Grade report card twice a year. The report looks at more than 3,000 acute-care hospitals across the country and assigns each hospital a letter grade, ranging from A to F, based on how well they protect patients from medical errors, accidents, injuries and infections.

11 Northwell hospitals earned an A grade In the group’s Spring 2025 report: Glen Cove Hospital, Huntington Hospital, Lenox Hill Hospital in Manhattan, Long Island Jewish Medical Center in New Hyde Park, LIJ Forest Hills, Mather Hospital in Port Jefferson, Northern Westchester Hospital in Mount Kisco, Phelps Hospital in Sleepy Hollow, Plainview Hospital, South Shore University Hospital in Bay Shore and Syosset Hospital.
Leapfrog evaluates hospitals using 22 measures that cover clinical processes, structural factors and patient outcomes such as infection prevention.
New York ranks 31st in the nation for the percentage of hospitals earning an A grade with a rate of 25 percent, below the national average. However, 73 percent of Northwell hospitals have earned an A.
Leapfrog also recognizes “Straight A” hospitals, which have earned an A grade in at least five consecutive reporting periods. Only about 12 percent of hospitals nationwide have achieved this honor, including three Northwell hospitals: Mather Hospital, Northern Westchester Hospital and South Shore University Hospital.

Patients who are treated at hospitals with higher safety grades are less likely to experience complications or injuries during their stay, according to Leapfrog. The group hopes its reports encourage hospitals to maintain high standards and continue improving care.
"All hospitals should be proud to earn an A, but they should not rest on that laurel," said Leah Binder, president and CEO of Leapfrog Group, in a release. "Patient safety is a relentless, never-ending quest to put patients first."
