Cathy Last, 50, of Frances Street, allegedly signed another woman's name to a prescription log to obtain Percocet and Atterall at CVS Pharmacies in Bellmore and Syosset and a Rite Aide Pharmacy in Bellmore on June 13. The indentity theft victim reported the crime to the Seventh Squad on June 17, triggering an investigation that led to Last's arrest on June 26.
Last was charged with felony counts of forgery, identity theft and falsifying business records. She was jailed in East Meadow over the weekend, and had not posted bail set at $15,000 cash/$7,500 bond as of Monday morning, according to court records. She was due back in court on Tuesday at press time.
