Martin Avenue Elementary School kindergarten registration is set for Friday, Feb. 9 from 9:30 to 11:30 a.m. and from 12:30 to 2:30 p.m. Registration is to take place in the Martin Avenue all-purpose room.
Children who will turn 5 years old on or before Dec. 1, 2001 residing in the Martin Avenue attendance area may be registered at that time. Registration materials may be picked up before Feb. 9 in the school's administrative offices.
When registering a child for kindergarten, parents must bring the child's original birth certificate and proof of residency. Any two of the following documents will be accepted as legal proof of residency: driver's license of parent or guardian, mortgage document, rental/lease document, telephone bill, utility bill, updated voter registration card, most recent income-tax document.
Additionally, parent must present proof of immunization signed and stamped by the child's physician.
